After filling out this form, the MCDC owner will contact your shortly in working out any details. 
No food or drinks allowed at the dance center with out permission with the exception of bottle water. 
All persons, including self, is at the responsibility of the renter. No liability will fall into the dance center. Insurance is preferred but not required. The renter will assume all risks and will do so by agreeing at the bottom of the form. A PayPal link will be sent you before the day(s) of use. The payment must be paid before the day of use. The space will need to be left the way it was found and all tables, chairs, trash and floors must be cleaned before leaving. A MCDC representative will be on site to open up, close and answer any questions.
